Heart & Soul LLC provides Congregate Waiver Services for individuals who have been diagnosis with an intellectual disability. Services are offered in both residential and community based environments with training and assistance provided in areas of personal care, socialization, communication, community integration, behavior management, health, safety, domestic skills, money management and other daily living skills. Each individual shall receive a provision of supports based on the assessment and development of their Individualized Support Plan.

Our Staff consist of QMRP, QMHP, & LPN’S and together we have over 20 years of experience. We offer a team approach in working with residents with dual diagnosis. All Staff have BEHAVIOR MANAGEMENT TRAINING in TOVA, CPR/FIRST AID CERTIFIED, and MEDICATION MANAGEMENT, and most importantly we meet individuals at their level of functioning and we’re making a difference!

Program Goals

  1. To provide community residential services that offer a variety of choices and community options.
  2. To provide services designed to promote opportunities for community inclusion, and eliminate the barriers and stereotypes that often lead to the practice of exclusion.
  3. To promote an environment where the individual has a valued role as a member of their community instead of just being an individual living in the community with a disability.

Population Characteristics & Needs of Population Served:

  • Males and females 18 years of age and above, who have been diagnosed with an intellectual disability
  • Individuals who are ambulatory and individuals who requires the use of a wheel chair
  • Individuals who are visually impaired and/or hearing impaired
  • Additional supports can be provided for individuals who may have secondary intellectual needs to include;
    • Bi-polar disorder
    • Schizophrenia
    • Anxiety disorder

Staff to Individual Ratio:

Our staff to individual ratio is 1:3 for ambulatory individuals.  But, the ratio is flexible to adjustment based on each individual’s needs.

Our staff to individual ratio is 1:1 as needed for wheel chair bound individuals
