A Legacy of Compassion & Community
Heart & Soul LLC is a Virginia-based behavioral health, intellectual and developmental disability (IDD), residential, and community support organization with over 30 years of service throughout the Hampton Roads, Newport News, and Richmond communities.
Founded in July 1994 by Matthew Bradley Sr. and Bobbie Bradley, Heart & Soul LLC has remained committed to improving lives through compassionate care, supportive residential environments, behavioral health services, community integration, and person-centered supports that promote dignity, independence, stability, and quality of life.
What began as a mission to provide safe and caring environments for individuals who had nowhere else to turn has grown into a respected multi-service organization serving diverse populations with varying support needs across Virginia.

Comprehensive, person-centered support services spanning residential care, behavioral health, IDD supports, and community integration.
Assisted Living Services
Compassionate 24-hour supervised residential care for seniors and adults, offering medication administration, ADL assistance, meals, recreational activities, and structured wellness support.
Connections Sponsorship
Family-style residential supports for adults with intellectual and developmental disabilities, promoting dignity, independence, community integration, and individualized person-centered care.
Community Excursions PSR
Psychosocial Rehabilitation Services supporting adults with serious mental health conditions through therapeutic community outings, socialization, coping skills, and social reintegration.
M.M.B.R. Skill Building
Individualized mental health skill-building services empowering adults with serious mental illness to develop life skills, emotional regulation, budgeting, and community stability supports.
Crispell Group Home
Structured residential supports for individuals with intellectual and developmental disabilities, designed to promote independence, dignity, daily living skills, and quality of life.
Independent Living Supports
Heart & Soul LLC also offers Independent Living Supports designed to assist individuals in strengthening daily living skills, maintaining housing stability, accessing community resources, and increasing independence within their communities. This is a community support service and is not a licensed DBHDS or VDSS program.

Matthew L. Bradley, Jr., LCSW, is a behavioral health executive and Licensed Clinical Social Worker with over two decades of experience in human services, clinical leadership, and community-based care. As Chief Operations Officer and Clinical Director of Heart & Soul LLC, he oversees the delivery of behavioral health, residential, and support services for individuals with mental health, intellectual, and developmental disabilities across the Hampton Roads region.

Heart & Soul LLC serves individuals across Virginia with service areas varying by program. Residential Group Home and Connections Sponsorship services are available throughout the Commonwealth of Virginia (statewide). Community Excursions PSR serves individuals within approximately a 20-mile radius of our program location. Assisted Living Services are provided in accordance with Virginia Department of Social Services (VDSS) regulations and licensure requirements. Contact us to confirm availability for your specific program of interest.
Heart & Soul LLC works with Medicaid waiver programs, DBHDS funding, VDSS residential funding, and other approved funding sources. Contact our office to discuss your specific situation and available options.
Referrals may be submitted through our online referral form above, by phone at (757) 504-2191, or by email. We accept referrals from CSBs, hospitals, healthcare providers, families, guardians, and self-referrals.
VDSS programs include our Assisted Living Facilities which provide structured residential care supervised by the Virginia Department of Social Services. DBHDS programs include our behavioral health, IDD, and community-based services regulated by the Virginia Department of Behavioral Health and Developmental Services. Both divisions operate under one unified Heart & Soul LLC brand.
Heart & Soul LLC does not provide transportation services. We assist individuals with coordinating and scheduling transportation services available through their Medicaid or insurance benefits when applicable. For our Community Excursions PSR program, our Heart & Soul Hummer is used exclusively for scheduled community outings and therapeutic group activities as part of the program — not as a transportation service.
